You are working in an emergency department. A 35-year-old woman arrives, complaining of nausea and vomiting, abdominal pain, fatigue, and frequent urination. You obtain her vital signs, weight, and height. Her body mass index (BMI) is 35%. She is not aware that she is considered “obese” by statistical and scientific standards. Suggestions, Obesity Case Study: Part 1 The following are some suggestions that you could consider as you begin assessment and treatment for this patient:  Obtain a pregnancy test (standard of care for all women of child-bearing age unless they have had a total hysterectomy).  Obtain a blood glucose level.  Obtain urine for ketone analysis.  Ask questions about stressors in her life.  Find out if she takes any medications, including prescription/OTC/ homeopathic.  Do NOT use the word “obese” but note to yourself that 40% of Americans have a BMI greater than 35%.  Ask about family history.  Ask about any past medical history on her part (including pregnancies and birthing children).  Ask her if she is diabetic. Gently discuss with her that she may be tired because she is carrying quite a bit of weight. Explain what obesity is, how it happens, acknowledge that obesity has become part of the American culture, and gently suggest that she also ask her physician for an EKG to see if the weight she is carrying is affecting her heart, which may be why she is tired. Also explain that stress can cause the body to hold onto weight. This topic must be handled with care when discussing it with a patient
